Rising fasting blood sugar levels

Hi. I am brand new to this site. I was diagnosed with T2D in 2013 when I was 50. I am now 60. Initially, a physician's assistant put me on Metformin and Glipizide. After he left, I started being cared for by a nurse practitioner who I have stayed with since. She looked at my A1c of 5.2 and decided that it was too low. So, she took me off of the Glipizide. Since then, my A1c has stayed around 6.2-6.4. Admittedly, I got so good at figuring out how to eat right in order to keep my blood sugar in a good range that I stopped testing regularly. Recently, however, I have been experiencing a very concerning trend. I will check my blood sugar in the morning and it will typically be in the 140s. I haven't been doing anything differently in my eating routine. In the past, if I waited a bit after getting a reading above 130, it would go down. However, in the past few months, if I wait longer to eat, it goes up. Typically, after I eat breakfast in the morning, my blood sugar goes down. I went to my nurse practitioner and asked if maybe I should finally be put back on Glipizide. She looked at my A1c, saw that it's at 6.8 and said "no". I asked her why and she said, "Because of the chance that you could bottom out." I said, "Then explain to me why my fasting number is going up in the morning." She said, "I don't know." I said, "I want to add that I have experimented with waiting a while after getting a high fasting level and it goes even higher. Explain that." She said, "I would need to do some research." Earlier, before I made dinner, I checked my blood sugar. It was 157. I decided to wait to eat. An hour and ten minutes later, it was 165. I decided that I needed to eat. I assumed that, the longer I waited, the higher it would go. I don't really know what to do. It sounds like my nurse practitioner is in over her head. She's only looking at a good A1c and suggests that maybe I go for a walk in the evenings. Given what is going with my fasting blood sugar levels, I don't think it would be a bad idea to add Glipizide back. I'm currently on Metformin ER 500mg, two pills (1000mg total) before bed. Your thoughts would be much appreciated. Thanks!

  1. , thanks for your question! Diabetes can be so puzzling. It can also be frustrating when doctors only seem to focus on A1c, especially when it can be very helpful to be proactive to noticing changes and trends to your blood sugar levels. Would you ever be able to seek another opinion from a different healthcare professional? It might be helpful to hear another perspective.


    What did your blood sugar do after you decided to eat dinner? Did it go back down or keep rising? Since you mention your morning numbers being high as well, have you ever heard of the dawn phenomenon?: https://type2diabetes.com/video/dawn-phenomenon. It might be something to look into as well.
