Understanding relationship between diabetic related-health knowledge,psychosocial self-efficacy and diabetic control

Hello. I am a doctoral learner under the direction of Dr John Bryan in the College of Doctoral Studies at Grand Canyon University. My name is Christiana E. I am conducting a research study to understand the relationship between diabetic-related health knowledge, psychosocial self-efficacy, and diabetic control.

INCLUSION- I am recruiting individuals that meet these criteria:
• aged between 18-65 years.
• Diagnosed with Type 2 Diabetes 6 months ago and above.
• Live in northern California.
• Supervised by a Licensed Physician.
• Taking 2-3 diabetic medications.
• Male, Female, and others are inclusive.

EXCLUSION- You cannot be in this study if:

• You are not diagnosed with type 2 diabetes.
• Younger than 18 or older than 65.
• Not living in northern California.

The ACTIVITIES for this research project will include:
• You are being asked to participate in the study by completing a survey questionnaire that will take approximately 20-30 minutes to complete.
• The survey will be open between November 6, 2023, and December 6, 2023.
• Your participation in this research is voluntary, and there is no financial cost or payment for participating.
• You can leave the survey at any time without any penalty.
• Data will be collected using the Survey Monkey platform via a survey link online from your computer.
• Please note that this study is not expected to pose any risk to you. If you choose to participate. You will not provide your name, place of work, or any other identifiers in the survey responses. Therefore, your responses cannot be linked back to you.
• You will be required to agree to an informed consent form that confirms your approval to participate in the study.
• If you wish to participate in this research, please click on the link below, which will take you to the informed consent form that will confirm your agreement of participation, followed by the survey to be completed.
• All data in this study will be protected with a password.
• Data will not be released to anyone for any reason.
• No name or personal identity is required for data collection.
• Raw data will be protected electronically with a password in a
protected USB drive.
•There will be no link back to responses from the participants.

•The chair will be given a password for access.

Your participation in this study is voluntary, and there is no financial cost or payment for participating.
All data in this study will be protected by a password and will not be released to anyone for any reason. Raw data will be saved in a Zip envelope with a lock and key.
